What should be reported here?

Adverse Drug Reactions (ADR)

Any noxious or unintended effect that occurs after the use of a medication.

Lack of effectiveness

When the medication does not produce the expected therapeutic effect (Therapeutic Failure).

Medication errors

Preventable incidents in prescription, dispensing, or administration.
